If you are visiting our office for the first time, there are a few things we need to know about you so we can begin addressing your dental needs. Please complete the four patient forms below and submit back to us in advance of your appointment so that we can begin getting to know all about you and your specific dental concerns. We’ll be ready for you when you arrive for your first visit to our office.

Amherst Dentistry accepts a number of different insurance options to cover your treatment at the time of service. Insurance policies vary, so we cannot guarantee that we will accept your insurance plan. We can promise you that we will bill insurance companies for your treatment. We give companies 45 days to render payment. Patients are responsible for the full balance after 45 days if it has not been paid by the insurance company. .

If you have questions about your insurance claim and whether or not your treatment is fully covered, you’re welcome to contact our helpful team and we can try our best to address your concerns.

The following are the only insurances we accept as of 2024: Blue Cross Blue Shield, CESA, Delta Dental, Dental Pay Plus, GEHA, Guardian, Lincoln, Metlife, Nova, Principal, and United Concordia.

Our payment options include cash, checks, Visa, MasterCard, Discover, and American Express. We also offer convenient, low monthly payments from CareCredit, the nation’s leading patient payment program. CareCredit can help you with a low monthly payment plan for fees ranging from $300 to $25,000. .

Go to CareCredit’s website and fill out an application for a medical payment plan. If you are approved, you can pay for treatments over time, including deductibles and treatment not covered by insurance. You don’t have to worry about reapplying or annual fees.
